If you are aged 45-74 and wish to receive a free FOBT screening kit, please complete your information in the form below. 

If you are aged 40-44 and would like us to arrange test when you turn 45, please also complete your details here. We will complete an eligibility check with NBCSP and if eligible we will arrange for an FOBT test to be sent directly to you.  If you are not currently eligible, we will advise you and place you on a recall to have a kit sent to you when you are.

All sections must be completed for eligibility check, and must match Medicare records.  Please ensure Medicare has your current address before completing this form. 

We will not use your information for any purposes other than to check your eligibility with NBCSP and to send your FOBT kit if you are eligible.
